Liquid Personnel is currently recruiting for a Social Worker to join a busy Adults Team based in Bridgend.

The successful candidate will be responsible for providing high-quality care and support to adults in the community, in accordance with the principles of the Social Services and Wellbeing Act.

Your main responsibilities as a Social Worker:

• Carry out comprehensive assessments to determine individuals' needs, ensuring all assessments are in line with the principles of the Social Services and Wellbeing Act.
• Collaborate with multidisciplinary teams to develop and implement personalized care plans that meet the unique needs of each individual.
• Assist individuals in accessing a range of community resources to support their wellbeing and independence.
• Conduct capacity assessments and address best interests, ensuring that all decisions made are in the best interest of the individual.
• Advocate for the rights and services of individuals, ensuring ethical and legal compliance at all times.
• Provide support to carers, offering guidance and resources to help them in their role.
• Actively participate in multidisciplinary team meetings, contributing to discussions and decision-making processes.

What we are looking for in a Social Worker:

• Social Care Wales registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
