Social Worker, up to £32ph, FRT, Merton
Pertemps are recruiting for a qualified and experienced agency Social Worker to join a busy fast-paced Adults First Response Team.
This role will involve working from the Civic Centre for the first 2 weeks. Hybrid working for duty at least 1 to 2 days per week.
The successful candidate must have experience of Care Act assessments, NHS Continuing Healthcare assessments, Safeguarding Adults Enquiries in a community setting and be able to undertake assessment and care management functions including presenting to Outcomes Forum for funding decisions.
