Key Responsibilities:
- Conduct assessments to identify the needs of adults requiring social care support
- Develop, implement, and review personalized care plans
- Provide support and advice to adults and their families, ensuring their needs are met
- Work collaboratively with multi-disciplinary teams, community organizations, and external agencies
- Maintain accurate and up-to-date records in compliance with regulatory requirements
- Promote independence, dignity, and choice for adults in all aspects of their care
Qualifications:
- Relevant qualifications in social work and registration with social work England
- Experience in adult social care, preferably within a local authority or similar setting
- Driver's license and a car
