Purpose of the Job: As a Qualified Social Worker /Senior Practitioner in the Adolescent Team, you will manage a varied caseload of adolescents, providing comprehensive assessments, planning, implementation, and evaluation of actions to promote their welfare. You will work collaboratively with multi-disciplinary teams to address the complex needs of young people, ensuring they receive the support and guidance necessary to thrive.

Key Responsibilities:

• Conducting thorough assessments of adolescents and their families
• Developing and implementing effective care plans tailored to individual needs
• Providing direct support and intervention to young people
• Collaborating with schools, healthcare professionals, and external agencies
• Maintaining accurate and detailed case records
• Advocating for the best interests and rights of adolescents
• Participating in team meetings, case discussions, and ongoing professional development

Requirements:

• Qualified Social Worker with current SWE registration
• Experience in children's social work, particularly with adolescents
• Strong assessment, planning, and organisational skills
• Excellent communication and interpersonal abilities
• Commitment to safeguarding and promoting the welfare of young people
• Ability to work effectively within a multi-disciplinary team
