Health & Wellbeing Co-Ordinator | Wirral | 37.5 hours per week | 4 on, 4 off rolling shift pattern 07.55am — 19.00pm | £25,125.00 — £29,677.44

For over 55 years, our client has provided safe, quality accommodation, along with first class support services to the most vulnerable members of society.

They provide person centred, supported housing across Wirral, Cheshire and Warrington, with a focus on specialist support services to people aged 18 years and over who are at risk of, about to become, or are homeless, and 16 to 21 year old care leavers.

You will provide high quality, tailored support that sustains the health and wellbeing of residents, incorporating the key principles set by the regulating bodies of supported accommodation for 16 and 17 year old Looked After Children and Care Leavers.

  • Capacity to organise and motivate
  • Interpersonal skills
  • Communication skills
  • Can deliver written and verbal reports
  • NVQ Level 3 or above in Health and Social Care or equivalent
  • Experience of working with and knowledge around Mental Health issues
  • Experience of working with and knowledge around Substance Misuse
  • Experience of working with and knowledge of other organisations and agencies

What will your role look like?

Your key tasks will be:

  • To be the single point of contact (SPOC) in the co-ordination and the effective delivery of Health and Wellbeing priorities for residents residing in the Links accommodation
  • To support initiatives to actively promote healthy lifestyles and improve social well-being for residents
  • To engage and empower residents to reduce the impact of risk factors that result in poor health
  • To promote protective factors using models that prepare residents for active citizenship
  • To promote and contribute to all Association initiatives that creates supportive environments
  • For Ofsted registered sites, to adhere to all Ofsted regulations and standards, provide quality and consistency of service and property standards to continually meet the Ofsted regulations, and be part of the preparation for Ofsted inspections.

What can you expect in return?

  • The pension scheme for new staff will be based on a defined employer contribution of 3% and employee contribution of 5% (minimum)
  • 5.6 weeks and maximum of 5 Extra Company Days awarded based on length of service.
