Responsibilities:
- Supporting tenants to access support, care and health services
- Maintain regular contact with tenants, implementing support plans and undertaking appropriate support actions
Requirements:
- Experience working with vulnerable adults with complex needs and/or challenging behaviours
- Understanding of benefits / housing benefits systems
This role is based out of our main supported housing property in Shepton, but will hold a caseload of up to ten clients across Mendip
