Position Title: Band 6 Social Care Worker

Role Summary:

To undertake person-centred assessments that promote respect, dignity, choice and independence. This includes risk assessments, mental capacity assessments and contributing to multi - professional assessments when appropriate

• To be able to assess and develop plans of care to meet the complex needs of service users with a variety of conditions taking account of risk and safeguarding issues.

• To work collaboratively within a multi-agency / multi-professional setting with relevant statutory agencies, health service colleagues, Police, as well as Third Sector organisations involved with supporting service users

. •1-2 years Post-Registration experience

•Excellent clinical reasoning skills, good understanding of patient flow, acute pressures, community rehab and related pathways

•To be able to effectively communicate with colleagues, peers, senior managers and clinical leads within the Trust.

Skills:
To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols. The post holder will have Current Effective Status on the professional register.

Qualifications and Experience:

• Education: Degree in Social Work or a related field.
• Experience: Minimum of 3 years of experience in a social care setting, preferably within healthcare.
• Registration: Must be registered with the Health and Care Professions Council (HCPC) or equivalent.
• Skills: Strong assessment, planning, and intervention skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team.
