Staff Partners are currently looking for a Band 6 Senior Mental Health Practitioner with a passion for delivering outstanding care.
We are supporting an NHS Trust in Cornwall covering all aspects of Mental Health. At Staff Partners we have nationwide contracts with some of the largest, and most highly regarded private institutions and NHS trusts in the country. Because of this, we are looking for enthusiastic Band 6 Senior Mental Health Practitioners to begin picking up work as soon as possible.
Job Overview
- To be responsible for the assessment of care needs and clinical management of a defined caseload within the NHS Trust Mental Health Service. •
- To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
- • To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
