Hackney Borough Council are seeking an experienced Hospital Discharge Social Worker to join the Out of Borough Team. The primary responsibility of this role is to facilitate the discharge of Hackney residents who are inpatients at hospitals located outside of the Hackney borough, within London.

Key Responsibilities:
Discharge Coordination: Manage and oversee the discharge process for Hackney residents admitted to hospitals outside the borough, ensuring that appropriate care plans are in place. - **Remote and
On-Site Assessments: While a significant portion of assessments can be conducted remotely, certain cases requiring a Mental Capacity Assessment may necessitate an on-site hospital visit. There are occasions where these assessments can also be completed remotely, depending on the case specifics.
Post-Discharge Review: Social workers are responsible for scheduling and conducting follow-up home visits to review residents well-being 4 to 6 weeks after discharge. Where appropriate, these visits should be coordinated to optimise time spent in the community. Simple reviews may be conducted via telephone, depending on the individual case.

Remote Work Option: The majority of staff in this role work fully remotely, with the option to work from our office located in Hackney Central if preferred.

Location: Primarily remote with some fieldwork as required. This position offers the flexibility to manage cases remotely while ensuring high-quality care and support for our residents. The role is vital to ensuring that discharged patients receive the necessary follow-up care, whether through home visits or telephonic reviews.
