Center Healthcare is looking for compassionate, dedicated, and experienced Community Mental Health Nurses / CPNs to join our growing team. As a valued member of our team, you will be placed in a variety of assignments within our clients' settings, providing exceptional nursing care to patients.

Key Responsibilities:

• Assess, plan, implement, and evaluate nursing care for patients in their homes or community settings, in line with individual care plans and clinical guidelines.
• Collaborate with multidisciplinary teams, including GPs, social workers, and other healthcare professionals, to ensure the delivery of patient-centered care.
• Monitor and maintain accurate records of patient care, updating care plans as required.
• Provide support, guidance, and education to patients and their families to promote self-care and independence.
• Adhere to relevant legislation, policies, and procedures to maintain the highest standards of nursing practice.

Requirements:

• Valid NMC registration as a Registered Nurse (Mental Health/Adult/General) with the right to work in the UK.
• Minimum 1-year experience in Community Mental Health nursing or a similar setting.
• Excellent communication, interpersonal, and organisational skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• A commitment to continuous professional development and maintaining up-to-date knowledge of nursing best practices.
