Job Description:
Sefton Metropolitan Borough Council is seeking a dedicated Adults Social Worker to join our Learning Disability & Autism Team. This is an exciting opportunity to make a meaningful impact in the lives of adults with learning disabilities and autism within the Sefton community.

Key Responsibilities:

• Conduct comprehensive assessments, care planning, and regular reviews.
• Collaborate effectively with individuals, families, and multi-agency teams.
• Support adults with learning disabilities and autism to achieve their goals and enhance their quality of life.

Requirements:

• Qualified Social Worker (Degree in Social Work or equivalent).
• Registration with Social Work England.
• Experience working within Learning Disability and/or Autism teams is desirable.
