The Integrated Independence Team (IIT) is an integrated health and social care service. The service places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system. The service will have a single point of access and use a multi-disciplinary assessment and care planning framework. The service will respond swiftly to service user need and, where possible, will treat people in their own home or normal place of residence. The service will also provide short term bed based intervention as appropriate. This will be in partnership with individuals, carers, professional staff in other agencies and service providers to establish arrangements and services that meet their assessed needs and promote independence, choice and control. The post holder will operate within the agreed performance management and budgetary frameworks. To manage a caseload of individuals and carers who require additional support or guidance in order to identify their needs and who may need close monitoring or frequent reviews and interventions to minimise and manage risk or where best interest decisions may be required. To provide assistance, guidance and direction to colleagues and assist senior colleagues and managers as required. To adhere to the standards of proficiency set by the Health and Care Professional Council so that registration can be maintained and learning or development needs are duly accounted for in supervision and appraisals. The activity of the post holder and his/her teams has a significant impact on the Councils performance rating and the external reputation of both the Council and the Services.

Must have at least 2 years post qualified experience. 

Must have HCPC registration 

 THE WEEKLY WORKING HOURS ARE 36 HRS A WEEK WHICH IS FULLTIME (MON-FRI) THERE IS REMOTE WORKING FOR AT LEAST 2 DAYS A WEEK PAY RATE £32.00
